Issue #1:
The quiz is asking for an access code/password.
Solution: The main cause for the quiz asking for an access code or password is the Proctorio extension is not downloaded or is not being detected by your browser. You can download Proctorio Here . If you already have Proctorio installed in your browser, please try uninstalling and reinstalling the browser extension.
Issue #2:
When clicking the Begin button in the quiz, nothing happens:

Disable other browser extensions that are installed:
Ensure your browser is updated:
High contrast browser extensions/settings have been known to cause issues with Proctorio functioning correctly. Please disable any high contrast settings you have enabled in your browser before taking a Proctorio quiz.
Issue #3:
After clicking 'Begin' for a quiz, a message pops up that says "Waiting for Canvas" or "Loading".
Solution: Try clicking the back arrow in your browser to return to the previous screen and then click the 'Begin' button again. If that doesn't work, restart your computer and try the quiz again
