Resolution Description

Select Create.
Select Multiple Choice as the question type.
Enter the question and list of answers.
Select Create.
From the Configure panel on the right side of the screen, select Response settings.
Change the Each person may respond option to as many times as they like.
Leave the Each option may be selected option to up to 1 time.
Select Activate to launch your poll.
